Before your first visit, please take a few easy steps:

1. Complete Your Forms

• Click on the forms below to open and download them.
• You can either:
  • Fill them out on your computer and email them to [email protected], 

OR

Print them, fill them out by hand, and bring them with you to your appointment.

2. Send Us Your Medical Records

• If another doctor referred you to us, please ask their office to send your medical records.
• If you were recently in a hospital other than Abington Memorial or Abington Lansdale, and you’re coming for a follow-up, please also request your hospital records.
• To make it easier, you can print and complete our “Records Request Form – To AMS” and give it to your doctor or hospital.
• Have your records faxed to us at 215-376-1705.
